I would consult a doctor or physician first.
An orthopaedic doctor would be the one you are after,if the condition is more serious then your personal doctor will refer you to a specialist which deals exclusively with hands.

A dermatologist specialises in skin care, fingernails complaints could be addressed by seeing a dermatologist.
I would go see a hand doctor. That is what they specialize in. I don't see how they cant help with fingernails.
